Membership Application Form
The Membership Application Form must be printed off, completed, signed and dated, and mailed to the address located at the bottom of the form. Included, along with the form, must be a $5 cheque or money order made payable to the West Prince Arts Council. This is to cover the annual, renewable membership cost.

Grant Program Application Form
The Grant Program Application Form provides you with a choice between a printable version and an on-line version. The print version must be printed, completed in full and mailed to the address at the bottom of the form. To complete the on-line form, click on the On-Line Form link below.

Primary Funding Source Form
The Primary Funding Form has been created in conjunction with the Grant Program Application Form. It is important to note that the funding cheque will not be released until a completed, signed and dated, copy of the Primary Funding Form has been received by the West Prince Arts Council.
