To fill out the on-line form, click once inside a box and type the appropriate information into the box. Once you finish entering the information into the box, click inside the next box. To insert the start and end dates, click on the down arrow next to the month, day and year and select the appropriate option from the list until you have the correct start and end dates appearing in the boxes.
